Preventing or removing creases/folds in fabric?

So I'm a hoop-girl, but recently for some reason, I've been having trouble getting the creases out of my fabric. I have tried taking off the hoop whenever I'm not working on it, but that doesn't help with most fabrics (such as aida) that I use. I'm considering getting a scroll frame (and will likely ask for a millennium frame for my birthday), but I do have a project coming up that requires 36" wide fabric. I have not found scroll frames that large, and it would be difficult to work with without a stand (and where I stitch does not have room for a stand). As for working in hand, I'm a cross-country stitcher and it's difficult to get the thread at just the right tension when I tried doing in hand.
How else could I prevent creases? How would I go about removing these? Even a gentle bit of ironing doesn't get them out completely. I've also tried pieces in the washer then hung them up to dry. Any other clue on how to prevent/remove the creases?
