Wanted-Teddybear chart

Hi, I'm looking for this teddybear chart. Does anyone know where I can find the chart for this. The picture is not to good, but i hope it works.

It is very big, and would look great in the nursery.

A childhood friend of mine had it on the wall, but I haven't spoken to her in years. I don't know where she lives. I have no idea who the designer or the publisher is.
