Is anyone familiar with this Shepherd's Bush chart?

I saw this ornament posted on someone's blog & fell in love with the design. I wrote to the blog owner who told me where she thought she had purchased it. However, when I contacted that company they did not recognize the design. It is definitely a Shepherd's Bush design but for some reason SB does not have email? Before printing it out & mailing it to them to ask if I can purchase it directly, I thought I would check here & see if anyone knows what the design is.

According to her blog, it is the Merry Merry Be kit. Hope this helps you track it down!

Apparently not available as a chart alone.
